Feeling a bit slower, more tired, or less active than you used to be? New research shows that these subtle signs, often written off as ‘just getting older’, may actually increase the risk of reflux.

A prospective study in Scientific Reports found that pre-frailty, the early stage of physical decline, significantly increases the risk of developing gastroesophageal reflux disease (GERD). And the risk increases as frailty becomes more pronounced.

What the study looked at

Drawing on data from over 327,000 middle-aged and older adults in the UK Biobank – a large, long-term health research database that tracks the genetics, health and lifestyle of half a million UK residents – researchers classified participants into three groups: non-frail, pre-frail and frail.

They used the widely recognised Fried frailty scale, which scores individuals based on:

  • Unintentional weight loss
  • Exhaustion
  • Weak grip strength
  • Slow walking pace
  • Low physical activity

The researchers enrolled participants who did not have GERD at the start of the study and tracked them for more than 13 years to see who developed the condition.

What the study found

Over the course of the follow-up period, more than 31,000 people developed GERD. When researchers analysed the data, they found that even those in the pre-frail group were significantly more likely to be affected. Compared to non-frail participants, pre-frail individuals had a 21 per cent higher risk of developing reflux. Among those who were classified as frail, the risk jumped to 60 per cent.

The risk rose in line with the number of frailty symptoms present. Every one-point increase on the five-point frailty scale was associated with an 18 per cent increase in GERD risk.

The researchers also examined each of the five frailty indicators individually. All factors raised the risk, but exhaustion emerged as the strongest predictor. Participants who reported persistent fatigue were significantly more likely to develop reflux, even after adjusting for other health and lifestyle factors.

This isn’t just about old age

Interestingly, the association between frailty and reflux was strongest in people under 60. That’s a key insight: early physical decline – often dismissed as simply feeling unfit or tired – may actually signal a link to long-term digestive issues, even in people not usually seen as at risk.

The connection was also more pronounced among women, the unemployed, those with low BMI, and people with higher levels of education.

What’s behind the link?

There’s no single explanation, but several overlapping mechanisms are likely at play. Frailty is associated with chronic low-grade inflammation, reduced muscle strength, and changes to the gut microbiome: all of which may contribute to reflux. Weakened diaphragm and oesophageal muscles can make it easier for stomach contents to flow upwards. While systemic inflammation may damage the protective lining of the oesophagus.

The findings also support previous research showing that loss of muscle mass (sarcopenia) is linked to GERD risk. Together, these results suggest that physical resilience, and not just diet, plays a key role in how the digestive system functions over time.

Why it matters

GERD is often managed reactively, when symptoms become disruptive. But this study highlights the value of paying attention to early, subtle health changes. Pre-frailty is a reversible condition, especially when identified early. Intervening with exercise, nutrition and lifestyle changes could help not just with physical decline, but also with the prevention of GERD.
