Spicy food, rich meals and alcohol have long been the usual suspects when it comes to triggering reflux. But new research suggests another, less obvious ingredient could be playing a role: glutamate.
Glutamate is a naturally occurring amino acid found in many everyday foods, particularly those high in protein or rich in umami flavour. Think soy sauce, aged cheese, tomatoes, mushrooms and meat.
It’s also the compound that gives processed foods their signature savoury taste, often in the form of monosodium glutamate (MSG). And while it’s widely consumed – and plays a number of important roles in the body – recent research suggests it could be contributing to gastroesophageal reflux disease (GERD).
What the research set out to do
In a large study, researchers from China analysed the relationship between amino acids and GERD using a method called Mendelian randomisation. Unlike observational studies, which can only show correlations, Mendelian randomisation uses genetic data to help uncover more reliable links between risk factors and disease. In short, it can reveal whether something like glutamate is likely to play a causal role in conditions like reflux.
The researchers looked at 20 different amino acids using data from two major European genetic databases: the UK Biobank and the FinnGen study in Finland. Together, these databases included genetic data from more than 750,000 individuals.
The standout finding
Researchers analysed 20 amino acids and found that only glutamate consistently showed a significantly increased risk of GERD. In the Finnish dataset, people with higher genetically predicted glutamate levels had a 17.5 per cent increased risk. In the UK dataset, the increased risk was nearly 40 per cent. When researchers combined the results from both datasets through a meta-analysis, they found that glutamate was associated with a 22.7 per cent increased risk of developing reflux.
The researchers applied multiple statistical corrections to ensure the results weren’t due to chance, and the direction of effect was consistent across all analyses: more glutamate, higher reflux risk.
Why might glutamate affect reflux?
The study didn’t just stop at showing a link. It also explored possible biological mechanisms, and there are several reasons why glutamate might play a role in triggering or worsening GERD symptoms.
First, glutamate affects the nervous system, including the nerves that control digestive function. It can influence how the lower oesophageal sphincter (LES) behaves; the muscular valve that normally prevents stomach acid from flowing back into the oesophagus. If this valve relaxes too often or too easily, reflux becomes more likely.
Second, glutamate may play a role in inflammation. The researchers suggest that it could activate immune cells in the oesophagus, increasing the production of inflammatory signals that damage tissue and worsen symptoms over time.
Third, glutamate may affect how we perceive pain. By stimulating certain receptors, it can make the oesophagus more sensitive… amplifying the burning or discomfort that comes with reflux, even when acid levels aren’t particularly high.
Finally, glutamate may influence gastric motility and acid secretion. For example, high-protein meals can slow gastric emptying in some people. This can increase pressure on the LES, again making reflux more likely.
What does this mean for people with GERD?
This study doesn’t suggest cutting out glutamate completely. After all, it’s a naturally occurring part of many healthy foods. But it does add weight to the idea that reflux is complex, and highly individual. What causes symptoms in one person may not affect another. And while glutamate isn’t commonly listed as a reflux trigger, that may be because its role has been underexplored until now.
